Primary Care Network Healthcare Team

Mental Health Practitioner

Michelle Tegama is our Mental Health Practitioner working together with the clinical teams to support our existing patient services in the practice.

Michelle is employed by Lancashire Care Foundation Trust and works across the PReston & South Ribble network practices and is based in Longton Health Centre on a Tuesday and Thursday

Michelle has a mix of face to face and telephone appointments and your triage requests may be signposted directly to her where appropriate rather than with a GP.

First Contact Physiotherapist

Sarah Fearnley is our in-house musculoskeletal first contact practitioner (or FCP) who works at the practice on behalf of Primary Care Physio for the Preston & South Ribble Network.

FCP's are trained specialists and can help patients with musculoskeletal issues such as back, neck and joint pain by:

  • assessing and diagnosing issues
  • giving expert advice on how best to manage their conditions
  • referring them to specialist services if necessary

If you contact the practice with back or joint pain, you may be booked in to see Sarah in the first instance rather than a GP. 

We have physiotherapy services based in the practice on a Monday, Wednesday & Friday

Please contact the practice using our online triage service for a first contact physiotherapy clinical assessment for any knee, back, joint or neck pain

Paramedic

Erica Leahy is our practice Paramedic who works for the practice on behalf of the Preston & South Ribble Primary Care network

Erica is working with our clinical team both in practice and in our community for acute on the day and routine conditions

If you contact the practice you may be booked in to see our Practice Paramedic in the first instance rather than a GP. 

Erica is based in the practice on a Monday, Tuesday, Thursday and a Friday

Clinical Pharmacist

Osman Hajat is our primary care network pharmacist working as part of our practice pharmacy team to carry out structured medication reviews for our patients both within the community and care home settings.

Our clinical pharmacist works as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly.  Patients may be offered a structured medication review (SMR).  A structured medication review (SMR) is a patient centred review of all medications, with the aim to optimise treatment and provide better patient outcomes.

Please contact the practice if you have any medication or prescription queries that can't be dealt with by your community pharmacist service.

Registered Nurse Associate

Jessica Kavanagh is our primary care network Registered Nurse Associate working as part of our practice nursing team providing patient care for our patients both within the community and care home settings

Jessica is in practice Tuesday, Wednesday, Thursday & Friday and offers both early and late access appointment times

Jessica is currently working towards her Registered Nurse Degree Apprenticeship Top-up training

Please book online for all nurse review and associated appointments including vaccinations and cervical screening appointments

Trainee Registered Nurse Associate

Ellie Fallows is our primary care network Trainee Nurse Associate formerly GP Assistant working as part of our practice nursing team providing patient care for our patients both within the community and care home settings

Ellie is in practice all week

Ellie is currently in her second year of training for Registered Nurse Associate Degree Apprenticeship Top-up training

Clinical Care Co-ordinator

Ruby-Lily Heywood is our primary care network care co-ordinator working as part of our practice wellbeing team providing patient care for our patients both within the community and care home settings

Ruby-Lily has appointments in practice each day

Please book online for all routine practice requested blood tests - we have limited hospital request blood test appointment availability and urgent on the day availability where required

Ruby-Lily carries out ECG clinics, Chaperone assists with Family Planning clinics and also INR clinics for patients who are on anti coagulation medication who require testing.

Ruby-Lily is trained to carry out health monitoring reviews, BP monitoring, height & weight monitoring, smoking and alcohol advice, memory assessments and NHS Health Checks

Health & Wellbeing Community Support Link Worker & Care Co-ordinator

Georgia Fallows is our Health & Wellbeing Community Support Link Worker / Care Co-ordinator in practice. 

Georgia works from Monday to Thursday in practice and carries out frailty assessments, Over 75 healthcare checks, Carer checks in face to face clinics or telephone appointments where appropriate to support patients if they have any unmet needs or simply need support from community services, of which she can help to signpost you to.

Georgia carries out Holistic Health Assessments in practice on a face to face basis - please get in touch if you would like to book an assessment

Georgia works as part of the Primary Care Network to support with social prescribing referrals where appropriate. 

Georgia will discuss patient needs on an individual basis to provide wellbeing or coaching support where needed

Georgia will signpost and link in with local services where appropriate via the South Ribble Council Social Prescribing Team 

South Ribble Council host a Social Prescribing Team who are there to support you and build confidence to manage your own health and wellbeing and address any other issues that may be impacting your day to day life.

If you prefer you can speak with a member of their team who will be able to understand if their service could help you, or whether they may be able to put you in touch with other services who are better placed to help.

The support the council provides is fully funded by the council and therefore free, confidential and personalised support that starts with you and can be guided by you.

Once you have contacted the council, someone will be in touch with you for a chat to get to know what they can help you with; from there they will sort out an appointment and support going forward.

The council want to support you in a way that works for you, so you will have the choice over whether you wish to have your appointments over the phone or in person.

Social prescriber intervention (sometimes know as community referral) works by providing support for any of the following issues:

  • money worries
  • housing issues
  • isolation / loneliness
  • employment / new skills
  • diet / exercise 
  • mental health and wellbeing
